Group ( 40K strong ), Chubb as the underwriter, The Federal Insurance Co. as the underwriter, Multi-Plan PPO,ie, choose your own doctor, 92% of doctors accept this plan,covers vision,dental, hearing,etc. This is nothing more than a discount card and may not be accepted by 92% of physicians.
