american medical and life insurance company pay insurance premium, need coverage---coverage denied philadelpia Pennsylvania

My husband and I have been paying premiums of $269 since 2010. In early September 2012 I had a cardio myopthay. AMLI has denied coverage for most of the claims in regards to this illness. They tell me that the hospital hasn't filed papers.They tell the hospital that I haven't sent paper work. At one point the billing clerk had to make them varify that they received forms. The emerengcy room $100 payment is the only one that they covered that wasn't $50 or less.

They say You can check all your information on their website, but when you go there it says the site isn't set for that yet.  We had insurance through Homeland Health that was good. ALMI claimed the coverage would stay the same. That was until we needed coverage.    Now we get 5 doctors visits per year@ $50 each.(this includes 5 total)$250 per year. And 3 diagnostic/ lab/ xray benefits @ $50 each$150. per year.  So we are paying $3228 per year and the bottom line is this company only covers a person for $400 in benifits a year. And I have to add  AMLI does not cover a womans mammogram!
