On March 7,2014, I mailed a rebate form to CMS Rebate Center and after 14 weeks I have not received my $50 rebate.

I just want to thank Crystal at the rebate center for finding an answer to my problem which was not their problem but the US Postal service who returned the original check as we were having our mail held due to vacation. Problem is solved as of this time. Thank u RIPOFF.com.
