November 7, 2018 I participated in an online survey over the phone for Blue Cross Blue Shield I was told I will geta check for $100.00.I did not get a check I reported Focus Forward to blue Cross Blue Sheild health insurance of Michigan they told me they are not a part of them and will report them as a fraud for using their name.

On November 18, 2018 I get an two emails from focus forward the first is from product manager Dallas Stevens saying if you do not receive your incentive within 2 weeks to reach out by email. The second email I got on November 18, 2918 says focus forward sent me a $100.00 gift card to Amzon as a gift card, they did not know I have an Amazon account there are a bunch of links to click on.

I did not click on any of the links in the email I contacted Amazon was told by Amazon on the phone there is no $100.00 gift card from focus Forwrd in my account. I gave Amazon all the information on Focus Forward they are going to report that Focus Foward is a scam.

I was thinking this was a scam when the 4 weeks has come and gone I had to the 18 day of December 2018 to get my incentive pay of $100.00 check in the mail it never came. Then I get two emails in the same day that confirm Focus Forwaed is a fraud a scam base on the two emails.

Every time I call Focus forward number dial Dallas Stevens EXT 239 I get a woman voice message I filed a complaint against focus Forward with the Better business Bureau on November 18, 2018 too.
