Forsythe Data Equipment Faxed invoice to Kevin Batesman of requesting credit card info SAN DIEGO California

We received a fax four times on 10/25/2004 for an invoice dated 04/22/2004 in the amount of $6000.00. The fax indicated that we were receiving a list of business records randomly pulled to be sent to us directly for unlimited use. The bill to and ship to referenced a "Kevin Batesman, 759787 Central Avenue, Collon, UT 43565".

The fax requested that we sign the approval and supply our credit card information, as well as an evening phone number of card holder.

The second page of the fax was a Credit Application, requesting business and credit information. The credit application agreement states that by submitting this application "you authorize Forsythe Data Equipment CO. t o make enquiries (sp) to the banking, savings, business, and/or trade references you have supplied".

We discarded all copies of the fax after failed attempts to locate said company to inform of incorrect fax transmittal.
