In January 2010 I ordered a copy of my kid's birth certificate online from nyc.gov. I have had my name changed LEGALLY so I asked NYC Vital Records to change the mom's name on the birth certificate. I sent NYC the original name change court document, copies of current driver's licenses for both the person on the birth certificate and the mother of the person named on the birth certificate, an application for a corrected birth certificate and the money--$55. A year has gone by and still no corrected birth certificate. NYC did cash the checks--in March 2010. Every couple of months, the NYC office of Vital records sends us a form letter with a "miscellaneousreject reason." In the past year, I've written emails to Edna Timbers, Teresa Yasin, and Tracey Cornish at health.nyc.gov. I've written 2 letters to NYC Mayor Bloomberg, with no answer. I wrote a letter to former NY state Attorney General Andrew Cuomo, with no results. I've emailed New York City/Manhattan Family Court requesting assistance, which they refused, no explanation given. I've written to the NYCCLU and they declined to help, instead sending me 3 phone numbers--2 of them long distance, which was not helpful.I've written to New York City Health Commisioner Thomas Farley, and got back another form letter.I've complained to the Federal Trade Commission, and they blew me off.

If you contact a government agency, requesting a service --in this case a corrected birth certificate--and pay for the service, it shouldn't take a year for the government agency to provide the service. It's pretty straighforward. Change the mom's name on the birth certificate, and send a copy of the corrected birth certificate to the person named on the birth certificate. Oh, and I wrote to Michelle Obama too. She didn't answer.
