Recieved on 12 Jan 2015 sent via UPS.  Requires $20.00 fee made payable to P.D.A. Main Office to relase $1,800,000. to my name / CPN 00094727617 at my home address.  My signature is required on the form.   It addressed to me with my correct name and address.    

This official looking form is similar to the scam I received last week from Financial Reporting Services, P.O. Box 107 Lawrence, NY 11559, and which I reported to Ripoff Report.   Both these scams announce huge amounts of money that will be paid upon their recieving my $20.00.   Both scams may have located me through my frequent UPS and online purchases from Publishers Clearing House.  PCH product orderings include entries for a chance to win large amounts of money.  It is like gambling, which may be why these scammers have identified me as a gambler who is easy to trick in to sending $20.
