Vertaloc, Inc On my report I recently received from United Health Care ,my insurance provider, the company I am complaining about is identified as Envision Bracing. As shown on the report I just received in January 2018 from United Health Care, I note that Envision Bracing has filed a claim for $999.99 for braces sent to me. Jupiter Florida

THIS IS WHAT HAPPENED: On January 20, 2017, Vertaloc, Inc., 14930 Corporate Rd., S, Unit 17, Jupiter, FL 33478, mailed to me a box that contained 3 pieces of orthotic items. Box was shipped via Fed EX. Box arrived at my home shortly after that. The information was not sent by Envision Bracing to United Health Care until December 13, 2017. WHAT IS WRONG ABOUT ALL OF THIS: I did not request any orthotic braces, nor did I request any braces of any kind. I had had no contact with this company at all..... No doctor or medical person asked that I be sent these braces or any medical items. THE ITEMS JUST ARRIVED AT MY HOUSE WITHOUT ANY AUTHORIZATION OR ORDER FROM ANYONE! do not have a need for any braces, nor have I ever discussed with any doctor or medical person a need for braces of any kind. This is obviously a scam of the First Degree........ After the items arrived, I started to throw them away, but decided to put the box up, with items still in the plastic in which they were shipped. There is a phone number on the box I received: (855) 694-3552.....This may be Vertaloc's number in Jupiter, FL. Company sent me orthotic items that neither I nor anyone else requested.
