Diversified Consultants filed erroneous credit information after 12 years of perfect credit Jacksonville FL

Diversified Consultants placed negative information on MY credit report after an alleged bill in my name from Nov. 2006 for Dish TV in the amount of $496.  It is now Sept. of 2018.  This was twelve years ago!!   I have NEVER had Dish TV.  I pay my bills on time.  I was never notifed by Diversified consultants or Dish TV that they would affect my credit.  The only way I found out about this is that I have credit monitoring and thus found out immediately that someone reported negative information on my report.  Had I not had this credit monitoring I would have been happily going through life never knowing my credit score has plummeted from no fault of my own.  After calling the companies I have been asked to provide proof of where I was living in 2006.  I have gone to my personal banks, my credit card services, and my utility service providers for this information.  After 12 years none of them have retained records from this far back.  Even the FEDERAL GOVERNMENT does not ask us to keep tax records for 12 years.  I am totally innocent of this bill and refuse to pay for something I did not do.  I am demanding to have this off my record.
