Submitted before deadline a request for 7 bottles of wine @ $3 each.  After waiting the 8 weeks, I find a phone number (none was on the rebate certificate) and called.  Was told to fax the missing items (which I had copied) but now I was beyond the deadline (which was only a couple weeks after purchase-making it impossible to contact after 8 weeks before the deadline passed).  It would be up to the vendor whether to honor my (no late) rebate request.

The address listed on the rebate form including the program ID/Offer ID for the promotion (this would be the Del Rio, TX address)
The day you faxed your copies
The agent's name to whose attention you sent the fax to.
