I worked for PI & I Motor Express for six months, it first started out that I was hurt while on the job out of state mind you. I was tarping a coil of steel when I caught the edge of the coil and sliced about a 2, 2 1/2 inch laceration on my upper arm. Not only did I have to go to the hospital and have stitches, I had to drive my load back to the terminal in Granite City without any pain meds and in extreme pain. I did not receive a dime for the incident and they also refused to pay the bills from the incident.
Beginning the first of the year they switched payroll companies and informed the employees that the first check of the year would be mailed and every one thereafter would be direct deposited. I waited two weeks for my first check and got a payroll advance to cover my house payment ($450), subsequently thinking that they would do like they normally do and deduct it from my paychecks.
However the next check came and they did not deduct the advance, so i spoke with my dispatcher and she said that I should wait until next weeks check to see if they deduct it then. Well then that check came and they still had not deducted it, so I deposited it in the bank, because three weeks into the first of the year they still had not set up direct deposit and the next day went to use my debit card only to find out that they had stopped payment on my payroll check.
So I contacted the big boss guy in Masury Ohio only to be told that I was going to be prosecuted for theft. WHY? I dont know. Due to the fact that they had stopped payment on the check I was unable to work for an entire week, so on the Friday of that week I quit. I turned in my fuel cards and keys and cleaned out my truck, which was at my home terminal without a load since I had not worked for a week.
I am currently unemployed and because of PI & I falsifying my DAC report I can not find another truck driver job at all. They have put on there that I misused company vehicle, which I only took my truck home on two occassions and had permission from my dispatcher to do so. They also said that I left under dispatch which was impossible since I had not worked for a week prior to my quitting. And they said that I misused company funds, which I did not have access to the companies money, only through comcheck and I had to have an authorization number in order to use them.
They eventually a month later paid me my last paycheck, charging me $200 for misc. Whatever that is supposed to be. When I contacted corporate they denied anything and said that the final charge was for stopping payment on my check. I got an advance for $450 and the company took $669 and some change and then said that I was the one that stole money.

You Need To Get Busy...

...by first stepping into the nearest state employment office.
You need to start by filing a complaint against them for failing to pay all the medical bills that occurred when you were hurt on the job. It does not matter that you were out of town at the time. They are obligated to pay them under worker's compensation laws.
You need to see if the State of Illinois will help you retrieve your unpaid wages, and any unauthorized deductions that they took from you.
You need to immediately begin the process of filing a challenge with USIS to have false and misleading information removed from your DAC report. You can go to the USIS website to obtain all information on how to challenge your report.
Do so in writing, each and every time you respond or send anything to USIS, and send each piece of mail certified and return receipt requested.
I would further sit down and send a certified letter to the company's headquarters reminding them know that willfully providing false and misleading information on a DAC report and failure to comply with the requirements of the FCRA can result in civil liability in the form of actual damages sustained by the applicant or employee, punitive damages (in the case of willful noncompliance with the FCRA), and imposition of costs and attorneys' fees.
Be firm, but polite in all that you state. Let them know that you intend in seeing to it that all letters of the law be followed, and that legal action will be taken if they refuse to mitigate your situation immediately.
You're in for an uphill fight, but you CAN get results if you start the process now, and follow it through to the bitter end. But see what assistance you can get from your state employment office FIRST, because you may be entitled to certain benefits you may not be aware of, as a result of the way this all went down.
Document everything, and don't be afraid to consult with an Attorney for assistance, who specializes in employment law.
One more thought on this. From what I read, there seems to be some issue with the main office not knowing what the office in Illinois was doing in regard to your advance that you took during the time that paychecks were held up.
You might attempt to find out who might be willing to listen to your entire side of things and maybe reason with someone to change this from the inside. Then again, you stated that you spoke to the "Head man"... so I don't know if that is possible or not.
If not...go after them. When you allow them to trash your reputation like this, it makes them think they can do it to everyone. The law is on your side. Fight them for what is right.
