Dentera Insurance Will not pay Alpharetta Georgia

The dental insurance is part of my retirment benifets the company changed company for dental insurance to Dentera, it took be more than 6 month with list after list of providers , and none took Dentera, each call I reminded them that I was in Texas, and out of network, finaly I was told I could use a dentist of my choice.

I had a lot of dental issues due to the length of time, sent out bill no payment, call Dentera needed one more pice of paper, never gave me a list I would get what they requested, one piece a time. The last time I picked up every thing the dentist had. Now they want a W9 from Dentist, and he unwilling to provide that, he does not want to be sign up as a provider.
