AJ Madison and New Leaf Service Contracts are working together on this scam. I firmly believe this. The salespeople at AJ Madison sell you the extended warranty package stating emphatically that you will receive 50% of the fee paid if you don't have any service calls on the appliances in 5 years. 

I am a very organized person and saved all of the information given to me from both companies after my purchase. It has been 5 years and I submitted my information to try and claim my refund only to be told by New Leaf Service Contracts that I need to submit the "original certificate" for my warranty. The thing is I never received any certificate, nor did any fine print ever say to ask for it or state that you must have this certificate to claim the refund.

I am so mad at both of these companies. I felt I made a good decision when I purchased the extended warranty only to find out 5 years later that the 50% back guarantee is completely bogus.

I deserve the deal that they made me when I paid them in good faith.
