I WAS SENT A IRS FORM 1099-C, REPORTING THAT DEBT WAS FORGIVEN FOR CEDIT CARD EXPENDITURE OF $2,143.82 BY DENT-A-MED. THEY WILL NOT STATE WHO IS SOURCE NOR

WHO IS FIRST CREDITOR OR WHAT IT WAS FOR!

I NEVER HAD A CARD IN THIS AMOUNT. AND THE COMPANY WILL NOT EXPLAIN WHO THEY ARE ACTING FOR!

AS THIS INCOME WOULD EFFECT MY DISABILITIES PAYMENTS, AND TAXES, I MUST HAVE AN ANSWER TO THIS LETTER.
