Purchased an Epson C80 printer from Circuit City Stores No. 3344 Aurora CO, 80012-4672 on 20 April 2002*. Phone 303 338 8867.

Was provided a rebate Redemption Form showing Price paid, program no. (KM001) and address of Epson Stylus $30 Rebate address, as shown above.

Program was for purchases made between 09/01/01 and 4/30/02*.

Envelope postmarked 23 April 02*.
"Redemption Company" refused to honer rebate offer.
Epson stole $30 from me.
