Forward Dental I intentionally missed my dental appointment because I was laid off. A few days after the missed appointment, I received a phone call from Foward Dental asking me to reschedule Milwaukee, Wisconsin

Ken,
Thank you for pointing out my error -- that's what happens when you are upset beyond reason. My report should read 'unintentionally' as I respect a business' time as they should (but don't always do) mine. I had so much whirling around in my life and a lot of due to my unemployment so I had overlooked my appointment. I hadn't known until I received the phone call from Forward Dental requesting I make another appointment.
There is more to this than what I have put down 'on paper' here, but I was trying to keep to the specific point at hand. So my error due to frustration lies deep.

You intentionally missed your dental appointment? The reason doesn't matter, you should have called them so they could schedule someone else in your time slot.
You cost them money, now they're costing you money, although it does sound like they made some poor assumptions about your dental insurance.
Sorry about your misfortune and lay off.
