Offered me a self employed person company type benefit insurance, took my money of $333.00 every month for almost 2 years, did not pay any of the claims, and left me high and dry with over $33,000 in medical bills. Am now bankrupt and about to be homeless because of this company.

You would be wise to ignore this total ignoramus. His/her mouth (typing) seems to just overtake any sense he/she MIGHT possess. Complete dolt, with NOTHING whatsoever to offer to the forum.

Did this insurance company violate the terms of the policy?

If they did, you should already have reported it to your states insurance commissioner. It would be a violation of insurance regulations (IF they did).
If they didn't, sounds like your policy didn't cover the circumstance. Not too unusual.
What did they tell you when they wouldn't pay? You forgot that part. They DID give you a reason, didn't they?
Sorry for your great financial difficulty.
