Infinite Vitality, Suncoast Total Healthcare Billed insurance company and charged me 2-3 years later! clearwater fl

I visited Suncoast in 2012 and started to recieve calls in the middle of 2014 (2 years later!) regarding $600 in charges I owed them, 2 years later.  I had not heard from them anytime in between.  I paid them every time I went in and asked them if I owed anything else.  I am not able to get a straight answer from them what I owe exactly and why.  I have been able to argue that I made a few HSA payments that weren't in there system, they double charged me for a few visits, etc.
