Philadelphia Sports Club Town Sport International Charged $54.50 for PT cancelled Nov. As of Feb 2015, charged $73.49 even though i am on silver sneakers Radnor Nationwide

After being a member for years and active with a personal trainer which cost 54.50 I canceled this in November and no longer saw anything on my bill. I am a member of Silver sneakers which service was offered at this gym. As of Feburary I am being charged 73.49 a month for membership. which is covered by my insurance plan keystone 65. I no longer have the Id tag with that number on it . but i am sure keystone 65 does. They are also being double billed.  It is now june they have robbed me of  357.45.I am now on hold with town sports for 24 minutes.Please help.
