Premier Health Plan MWI deducted money ($360.00) from checking account that wasn't authorized by me Omaha Nebraska

I found out that a company called Premier Health had been automatically deducting payments in the amount of $14.95 directly out of my checking account with out being authorized by me. It totalled to $360.00 !! I contacted my bank, they claim they are not responsable for the fees!

I called Premier Health they would hardly speak to me and of course had no answers. How can these companys break into our accounts with out providing signatures bank numbers ect... all steps that are always taken when money is automatically deducted from ANY account ? How do I get my lost stolen money back ?
