To file a claim, I was to provide the following information:

I get many such offers including this offer for a reward or inheritance or other major cash giveaway. Use common sense here. Often these are nothing more than identity theft scams. They start asking for the info you see. If you comply, they wilol try to build up your hopes a receiving a huge cash payoff. Eventually, they will require either a few hundred dollars to process your claim, and/or your social security number. Once they get your SSN, they use your personal info to get credit in your name and rob you blind. The processing fee is also gone.
Do be such a sucker, your own greed will sink you here.
