Recd official looking documents in the mail regarding a questionaire to be filled out to protect cuts to Social Security benefits. Attached letter states the form must be returned within 7 days, with a check for the amount of $17.00 to Mr. Gordon Neff. No physical address, web address, no telephone/fax number.
