Anthem - Letter of Evidence

After 6 letters to resolve the issue, below is the C.G.S. a legal statute that requests payment to me within 30 days. To date, it is not resolved. As a former state employee, when a visit to doctor is complete, the reimbursement is submitted by the doctor's office to reimburse me. I hope you can help me resolve it. Thank you. Susan Orlinski

RE: Anthem Reimbursements
Dear Anthem:
Fourth letter your company has not responded to:
I am writing to you because I have not received my reimbursement for a doctor’s visit and medication for 2012 occurred on 10.31.12 with Marlborough Family Practice. My insurance expired on October 31, 2012. I have forwarded bills and receipt for payment and have not heard anything. The bills and receipt I sent reflect that I did still have insurance through my current employer, State of Connecticut, Dept. of Mental Health, Southeastern Mental Health Authority until October 31, 2012. I no longer have insurance after November 1, 2012. I would like reimbursement as this has been going on for quite some time.
Enclosed please find Connecticut General Statutes – Volume 11: Listed under “TIME OF PAYMENT OF CLAIMS: “payment will be paid immediately upon receipt of due written proof of such loss” which was provided within two weeks of the visit (letter sent 11.25.12). Please forward a check for reimbursement immediately as this has been going on for quite some time since last year. There is nothing to argue over, money is due to me and I would like a check immediately. This is a serious matter.
Thank you for your help.
Sincerely,
Susan Orlinski
