In 2004 I deposited / set up an HSA account with them, though a representive in Durango, CO. I then recieved a IRS form 5498-SA for that tax year from the Fortis Insurance Company for the correct amount that I deposited.
Thinking that this was simular to a savings account and I just let it ride. I have been searching to get the answer as to where the funds are located for about 5 years. Only to be told that the HSA accounts were turned over to a different area of the company when they became Assurance. The insurance broker that I originaly was dealing with is no longer in buisness, but got a pretty wierd feeling when I reached out to a current agent in the same town.
It seems that Assurance should beable to help out but I have had no luck with them at this point in time.
