First mistake - I searched online for birth certificate.
Second mistake - I trusted what looked to be a legit site and filled out the form and paid the requested $47.00
First indication I was scammed - I received email stating the following:
"Congratulations, your payment has been successfully completed.
You have purchased through our secure online website, our preparation assistance services for a birth certificate application.
We would like to inform you, that this transaction will appear on your bank statement identified as "Vital Records".
Order summary:
Order Number: XXXXXXXXXX
Record Name: XXXXX XXXXX
Place of Birth/State: XXXXXXXXX, XX
Ship to information
Name: XXXXX XXXXX
Address: XXXX XX XXXXXXXX XXXX XXX XXXX
City/State/Zip: XXXXX XXXX XX XXXXX
Summary of Charges
Amount paid: $47
What’s next?
Our team of experts will review your application to confirm that the information provided is correct according to the state requirements.
Once the review is complete, you will receive your mistake-free application along with the personalized instructions so you can complete the process."
Wait! Hold on! So I can complete what process - you took my money - what other process is there to complete.
....wait for it...
Relization that I'd been had - email with a generic pdf with my information filled in telling me that in order to get my birth certificate I needed to print out and sign the pdf along with any other documents that the state required (see your states vital statistics department online or in person for this information), put it all in an envelope with any fees the state needs to process my request and - Just like that! The state would send me my birth certificate!
Complete and total scam!
Do not be fooled by this terrible rip-off!

With all due respect, there is no way that anyone could have (or can) receive a birth certificate from onlinevitalus.com. They are not affiliated with any vital records or other government offices and say as much in the fine print.
To whit: "O.V.U.S. (Onlinevitalus) © 2019. All rights reserved. Disclaimer: O.V.U.S. (Onlinevitalus) is a privately owned website that is not affiliated with, not owned or operated by th U.S. Government or any government agency. O.V.U.S. (Onlinevitalus) is an independent company that provides application preparation assistance services from $47, that will help you save time in any vital record application process.
Thanks to our online services, you will save a trip to the Health Department and will be able to easily fill out your application. If you are looking for blank forms, you can get them for free at the relevant state offices" Keyword: "preparer", yet oddly the large print above the button labeled "Birth Certificate" presented on their website clearly reads: "Vital Records" "Avoid waiting in line.
Get your Vital Record from the comfort of your home" "Order a Birth Certificate" This is presentation, at the very least, is misleading and disingenuous- and there is no way that someone could present this site, in this fashion, without doing so knowingly and willfully. Is it illegal? Probably not. Is it a scam and deserving of being called out for what it is on a site like ripoff report? Absolutely.

Just in case

Just in case someone needs to know my experience, I ordered a birth certificate with this company to submit my Medicare enrollment form. Everything was as promised. I don’t know what you had to deal with, but I personally received my certificate without any problem.
