P3 Health partners Medical Gropup Non-payment & fraud Henderson NV

I had an insurance coverage from P3 Medical Group from 2022 until April 2023.  Carolyn Dechaine was my Primary Physician at P3 Medical Group. Dr. Dechaine was coordinating my Diabetic Health problem with my cardiologist Dr. Miranda. Dr. Miranda  with the approval of Dr. Dechaine ordered an blood work  on February  27, 2023 at Quest diagnostic. When Quest diagnostic  submitted the bill to P3 Medical  Group, Quest Diagnostic informed me that P3 Medical Group declined to make the payment.  I was also informed that Dr. Dechaine  left  P3 Medical Group on March 8, 2023. Even though  the service was made on February 27, 2023 P3 does not want to acknowledge their financial obligation.  I reached out to P3 several times and they ignored my  letter.  They also claimed that my written complain was forwarded to their California office and that they don't have any obligation for this even though I have addressed my complain to their office at Henderson , Nevada. 

I informed them that I wanted to follow my Primary Physician. However, they also refused to give me the forwarding address of my Primary Physician, Dr. Dechaine.  Instead, without my knowledge and authorization, they asked  Humana who was my Primary Insurance to put a person with the name of Trent Levitt to be my Primary Physician.  I never met and do not know this person.  P3 placed this fraudulent name in order to make claim under my name for my Medicare coverage.
