I have been receiving forms informing me "office records now read that the holder of this document has been identified for the award payment data release as here stated in the total $amount 1,400,000.00.  The amount here typed is guaranteed as reported"


After reading the Document of AWARD PAYMENT, I realized they were not offering me anything for a prize, The Holder of This Document (my name) individual named upon this document shall now be proceeded a folows.


There is no phone # or e-mail address.  The address on the "send your money" envelope is:  Received from:  Known Individual , issuance office, proceedings Director, P.O. Box 9000, East Meadow, Ny 11554-9000
