This looks official printed on green cardstock. I looked up Form LC005 and could not find any information to that specific form. I also looked up Benefits Processing Center PO Box 1367, Durant, OK 74702 and saw there were hits saying scam and a 93 year old man reported this in 2013 saying it looks official, but beware. The company just wants more personal information on you.
