I had an HSA account with my employer. The card that was issued was from BenefitWallet a division of Xerox HR Solutions.   My card had a substantial balance and was still avtive through 2018.  During a routine Dr’s visit I presented the card for my co-payment and the card was denied.   I called the number on the card and was told that my account had been closed. They apparently made the decision to move away from Mastercard and to Visa. They said they contacted me and sent me a new card. Both of which did not happen.  They said that I could request a new card for a fee!   I told them to simply cancel the account and send me the balance. They agreed to do so for a FEE.

4 weeks later no check.  I called back to the company and again asked that they close the account and send me the balance. Again they agreed to do so. Two weeks later I get a call from the company asking me if I really wanted to close the account.  I said yes.

4 additional weeks go by and no check. I call again. They had no record of me requesting to cancel the account.  I have to go through the whole thing again.  In the meantime they had charged me two months’ work of Service Fees.
