There is no reason given for not paying the claims, or if there is, it is because they claim to have not recieved our Pre-existing Condition Form, although our policy specifically states that pre-existing conditions are not excluded. Despite that, we have mailed and faxed the form, and have the fax confirmations, with the following result:
