Centennial Medical Center Owned by Tenet Healthcare Corp. is a hospital located in Frisco Tx. The following is corrspondence between me and Centennial Medical Center and what happend to my Mother, I would advise
people not to use this Hospital if you want to live


1 On 01/31/09 at or about 8 P.M. 67 year old female Judith Pilcher is transported to Centennial Hospital by Frisco EMS feeling weak her blood sugar was in the 5's when Ems arrived

2 On or about 02/01/09 in the early hours of the morning she was admitted and put in ICU Sometime during that same 24 hour period they tried to move her down to a room but after standing up she almost passes out and is kept in ICU, but is moved to a room on 02/02/09

3 On 02/02/09 at about 4:00 to 6:00 p.m. my husband and two children visited my mother during dinner time, when her dinner tray was brought in it was non diabetic tray, I was very angry and brought it to the staffs attention
at that point she got a diabetic tray. At that point she was up and moving around going to bath room on her own. My husband and myself spoke to her nurse outside her room before leaving she stated to us she doing fine and
I'm sorry about the non diabetic tray

4. At around 10:30 p.m. on 02/02/09 she (Judith Pilcher) spoke to her friend on her cell phone from the hospital bed

5. 02/02/09 at 12:20 a.m. We received a call at 214-407-7444 at our home the house supervisor stated to me your mother has coded you need to get up here right away

6. 02/02/09 at or about 12:25 to 12:30 while I'm in route to the hospital my husband calls the hospital and is advised by the house supervisor his mother n law has passed away (HIPPA) violation While at the hospital the doctor that attend to my mother is on the phone with a person who he assumes is my husband, giving him all the details of my mother's death (HIPPA Violation) mean while later on early in the morning the house supervisor is offering to have her house keeper to come over to our
house to clean up? The funeral home is contacted in Shreveport Louisiana by my husband before I'm even aware of my mother's death (phone records and times of calls are available)

7. We had my mother's body sent to Osborne Funeral home in Shreveport Louisiana and is buried on 02/06/2009 in Forest Park Cemetery

8. 02/09/09 a certified letter comes from the United States Post Office addressed to Kelly Tubbs. It stated we have been trying to contact you by phone with no success please contact me so we can discuss your mother'shealth care from Krissa McCulla director of quality control

9. On or about 02/09 & 02/10 2009 I called at least a dozen time trying to contact Krissa to find out what was going on, finally I spoke to someone else in that department they advised us that auditors were in the hospital and she was tied up with them, within minutes she returned the call & advised us to come in and not to bring any type of recording devise and no attorneys period

10. on or about the same date we met with Jim Murphy, Krissa McCulla andthe doctors that was present during the death of Mrs. Pilcher. At which time Jim advised us that the heart monitor alarm was not on and nothing like that has ever happen and it was HE that found out about the problem and measures were being put in place to make sure that this never happens again, I asked him if he felt the hospital was at fault and he stated YES
however he denies that in a later meeting with Bill Henning CEO of the hospital ,but Jim & Bill both admit it was clearly against the hospital policy to have them turned off

11. on 02/22/09 my husband sent a email to Jim Murphy requesting a meeting with he and the CEO Bill Henning, on or about that week we all met I had a pocket recorder to record the meeting ( copy sent to Shell Cooley) in that meeting if you listen to the meeting(it's about 44 minutes long) listen carefully to the statement Mr. Murphy makes its 19 minutes and 47 seconds into the meeting He states if the monitors had not been turned off this probably would not have happened. It was clear the Hospital was negligence

12. A day or so later after 02/22/09 Krissa Mcculla called my Husband and advised him Michael Carter would be calling I was not aware of this call at this point
From this point on I had no commutation with the hospital or the attorneys until I spoke with the new outside attorney a few weeks ago

The following is what happened between Centennial's in-house attorneys and my husband in his own words and all without my knowledge

1 Michael Carter called me to advise me he was in touch with the hospital and would be taken things from here, he and I discussed the entire events surrounding Mrs. Pilchers death and wanted to know what we wanted
I advised him of amount that was discussed in the meeting * important to note he has never met me or my wife and had no documentation signed by Kelly giving him permission to discuss the issue's with me nor did he even know who he was speaking with (HIPPA Violation) the next day he called me and advised me that the hospital was prepared to settle this for $50,000 I
advised we had just spent a little over 10,000 on the burial along. He advised me to discuss this with Kelly and to get back with him. Knowing what we knew had happened and after speaking with some of my friends in other states that are medical doctors and one in Houston, they all advised us to first keep their name out of this but the medical records were the worst they
ever seen and it appeared the hospital was trying to hide something with all the corrections ,scratch outs and all the time discrepancies all around and just after Mrs. Pilchers death. Based on all this I knew Kelly would not accept that offer however Mr. Carter advised me in our next telephone conversation that $60,000 is the offer and that is it, they were self insured and had plenty of money to fight it and appeal it if they lost the case which would be their worst day in court and our best as he put it so we had better take it or else, he had the settlement drawn up and sent to my email as to say sign it.

I'm not an attorney and felt threaten by his remarks and worried for my family if I did not have her sign, Kelly did not know about the threats nor did I want to burden
her with this being her mom just passed away. So I had her sign the last page without her knowing what it was and told her she was getting 60,000 now and the rest later, about a month ago I found a negative mark on my
credit from Teneth health corp. collectors after investigating this I again spoke with Mr. Carter as I thought it had something to do with my mother in law in fact it had something to do with my son he said coming to the ER and that he would have that written off as a courtesy I found that very strange and that was the last I spoke with Mr. carter. Kelly states she has no valid settlement despite the check she received , In her words it's not one or two this about this tragedy that wrong it's a whole list

1. Many HIPPA Violations by the house supervisor, Doctors and

Attorneys
2.Serving a Diabetic a non Diabetic meal

3.Moving a sick person from ICU

4.Turning off the heart monitor alarm because they could not get a good read (per the CEO)

5.Medical records are a mess and looks like the hospital is hiding something

6.Threats made by michael Carter Teneths attorneys

7.Writing off a bill as a courtesy that we owe to keep us quite

8.Settlement is done with in less than 36 days ( VERY QUICK)

9.The meeting between the CEO, Mr. Murphy, My Husband and myself that recorded them admitting negligence

10.The fact the hospital did not want us to bring a attorney or recording device

11.Who really found out what happened to my mother auditors or Mr. Murphy as he states he did

12.The house supervisor offering her house keeper to help us out because she knew she was in violations of HIPPA rules and regulations

13.The fact the CEO is no longer there???

14.The attorneys never spoke to me about this settlement so I knew nothing about it and much more

For all I know my mother could have been murdered with all this uncertainty , your client needs to meet with me in person to settle this the right way once and for all, I feel I've up to this point been very amicable considering what has happen
