I Wah C Tang Dba Trusty Trucking, LLC made several inquires to this Ad posted on Craigslist-Ad said, looking for Driver-Class A CDL dated 04-06-2016.  Responded by CEO/Owner/Chairman Chrissy Alvarez Smith/Dianne M Sholl; Dba Diversified Transportation, Inc. located at 6212 South 75th Avenue Suite 4 Laveen, AZ 85339.  Communicating with Owner/CEO were emails, text messages and phone calls.  The contract were signed 04-26-2016; generated by CEO/Chrissy Alvarez Smith at her place of business.  The contract were to pick up five gaming machines at Bally's Technologies at 6601 South Bermuda Las Vegas NV 89119 by 04-28-2016.  The five gaming machines are to deiver to Potowatomi Bingo and Casino at 1721 West Canal St Milwaukee WI 53233 by 05-02-2016 09:00.  My company Trusty Trucking, LLC; delivered these gaming machines on the date indicated on the Bill of Lading with no issue.  The rates were $1.37 per mile loaded and $1.00 per mile empty agreed by both parties.  Company Check #7730 were written by CEO/Chrissy Alvarez Smith upon on my return for this amount $3865.39.  This company check #7730 was deposited on 05-13-2016 and returned to me by Wells Fargo Bank 05-14-2016 with "INSUFFICIENT FUNDS".  I am seeking restitution for my lost from Maricopa County Attorney's Office.  I am suing for 3 times the amount for punitive damages.  CEO/Owner Chrissy Alvarez Smith missed her court date in 09-15-2016.  There is a warrant for her arrest.  Do not do business with this person or her company.  YOU HAD BEEN WARN
