Fidelity (GAP 360) is supposed to cover copays and deductibles that are not covered, by your health insurance plan.


I had three medical bills I sent to the.  On each medical bill Fidelity requires the expiation of benefits (EOP) from your primary insurance company explaining how they paid the bill and how much you owe.

Fidelity also requires you get a copy of the original itemize bill with a diagnosis code that was sent to your insurance company:

I had three medical bills, so I got the three EOBs together, called each medical provider and got a copy of the original itemize bill and sent them in.

Fidelity Security Life Insurance Company denied the most expensive copy out of the group of three as not having a diagnosis code.

I found plain as day there was a diagnosis code on the copy of the bill I sent in.  

I called Fidelity and was told they had 10 pages from me, they were all EOBs.  No original bills with the diagnosis codes on them.  I would have to fax the one page in again.  

Thats odd since I had three (3) claims, two (2) of them had already been paid so they included original bills.  The other pages would have had to have been the original bills.  One included a couple of itemized pages, but that only totals seven pages.  It sounds like they had more than they were even supposed to have.

At any rate, Fidelity certainly is making it as inconvenient as possible to get reimbursed on claims.  Few people with a full time job would have the time to fight with them as much as it takes to get these claims paid.  Perhaps thats the idea
