Gary Smith (physician group of arizona) came in for annual physical checkup ... code my lab work as a diagnostic GILBERT AZ

I went in to see Dr Smith for a physical checkup on 6/23/17. After talking to Dr Smith he tell me he need to draw some blood work. I ask him if its covered... he said Yes .. stating that its part of annual physical exam. Month later i got a statement from insurance that the office visit was covered 100%. Weeks after i got another statement showing i owe for lab work @ Sonora quest. I contact Dr Smith's office and someone told me they will resubmit code to insurance. A month later i saw another statement the balance has reduce but not 100%. I then contacted the office and someone there promised to fix the code and resubmit. Its April 2018 not only the problem is not fix but now i got a statement from Sonora quest that i will be up for collection. My remaining balance is $30.69. I've been calling Dr Smith office spoke to Dawn, my insurance, and Sonora quest and neither one is helping me resolve this issue. one of my lab work was billed as diagnostic while the rest is preventive. the office visit was billed as preventive.  Until doctor office resubmitt  a preventive code to healthnet ... healthnet will not pay sonora quest.  Sonora quest is putting me up for collection end of month.
