Humana Ins. Insurance rip off. Lexington Kentucky

I have my health insurance with Humana with prescription drug coverage. I needed several shots from my Dr. and they refused to pay them. My mediacal card says I have the coverage and I have a 45.00 deductable. I recived a bill from the doctor for over 400.00 for the two shots. Humana did not pay there part of the bill. They finally sent me a check for 90.00 that was what thay say is their part of the bill. If you deduct the 135.00 from the 416.00 you have 218.00 left to pay. They refuse to pay that. They also have keep sending me Medicare prescription drug claim forms which I have filled out and they keep refusing to make a payment, saying that the 90.00 they sent me was all they were going to pay.
