Jackson Hewitt Tax Prep David Edwards, preparer; no longer employed at this location Did not include my Social Security form on 2012 tax return. Penalized $686 in 2014. Bradenton Florida

I received a lump sum retirement payout in 2012. Did not know what line to claim it on tax form, so I requested Jackson Hewitt prepare my taxes. I included all income forms. I paid $135 for tax prep. Recently I received notice from the IRS informing me that my Social Security income was not reported, and I owed the IRS hundreds of dollars. When I consulted Jackson Hewitt, they implied I did not include that form. Yet it was attached to my file copy. Regardless, I was responsible for the entire balance of their error. I may be reimbursed for the $19 penalty that the IRS tacked on. I received no apology. I am currently living on a Disability pension, and must make monthly payments to the IRS,which I cannot afford.
