I've been paying $40 a month for over a year and a half for dental insurance through Lincoln Financial Group. I got my wisdom teeth removed and attempted to send the claim to Lincoln Financial. First they told me that I needed to send the x-rays and all surgery info to my medical insurance, which I did. Then Lincoln sent me a letter explaining that in order for the claim to be accepted I need to send Lincoln a copy of my EOB (explanation of benefits), this seems unreasonable to me as Lincoln is the benefit provider so how is it that they don't know what benefits they provide to me? What the F have I been paying for? My claim has been denied because, apparently, this company is either really incredibly stupid (this includes everyone who works there) or, which actually seems more likely, Lincoln Financial Group is the epitome of low-life scum bag insurance companies and a major contributing factor to perpetuation of the downward spiral in the American Medical System.

submit your MEDICAL INSURANCE'S EOB

What Lincoln Financial wants is a copy of your MEDICAL insurance EOB. They want to know what your medical insurance paid so you don't get paid twice. Your medical probably didn't pay for everything, so your dental will cover (some of) the rest. The previous consumer who said your dentist has the EOB is wrong.

The EOB is a document your dentist should be able to provide. When your dentist office's staff is done processing your insurance and what you owe, they usually send this to you in the mail. It may take a few weeks to process, but contact your dentist for this document. Mail it into Lincoln Financial Group with a letter explaining the confusion and requesting reconsideration. You should have no problem getting the claim paid from there.
