PRAZA HEALTH iNTERNET SCAM. Free sample gets you unwanted product also and recurring bills over $87 per month (forever after) Scottsdale Arizona

I ordered a free trial sample of ZYNEV at $4.95 S & H. Declined order of CREO-Boost. Received 1 month supply each of ZYNEV and CREO-BOOST. I Was billed for $4.95 + $29.95 + $52.49 ($87.36). I used one of each product  for five days. On sixth day I passed blood in my urine and I stopped use. Bled for six more days, passed both blood and bood clots the next day. I Began bleeding again after a one & one-half day pause and then repeated the same cycle for two more weeks. I tried to cancel order at PRAZA HEALTH by phone. Custermer service would only offer 30% discount on all future mailings (I declined and said I did not want any of their products ever!). Then a 50% discount was offered (I declined again). I stated that only a full refund would be accepted. They gave me RMA numbers to return all unused product for a full refund but did not give an address for the return. I have disputed the charges with my credit card company and cancelled the accoun, but my card company is still billing me and adding penalty charges.
