Dear Premium Nutraceuticals:

I have just recently discovered that you have charged my BB&T credit card ending with XXXX twice on 12/04/12 and again on 01/03/13 for $89.99 each time.
 
As far as I am concerned, no whatsoever consent has been authorized by me.  You initially offered free trial samples then
turned around unexpectedly and subtly still charged my BB&T visa.  

This transaction you actually did was absolutely unauthorized and illegal.  I have reported this case to my bank after realizing that my credit card account was charged by your company for a total of  $179.88. After talking with Aimee from your office on 01/10/13, I have sent  the enhancement product called Vydox that I just received back to you with UPS Ground the same day with the tracking number Z3867390360904078. 

Therefore, please credit back to my credit card immediately for the amount $179.88 you charged without my agreement to straighten up this unfortunate incident.
