I filed three $10 rebates (limit was 5) for the Oral-B Triumph toothbrush and all three were denied because "According to the requirements of this offer, all requests must be made on an original order form." I did have the original form, but it smudged so I went to the website that was on the original form and printed it out again on an official rebate form.
I first called Proctor and Gamble who then referred me to their rebatee hotilne which is 1-800-979-8268. The woman said that when they get them in from a customer who printed them off the website, it looks like a copy and it often gets denied.
She fixed the denial and my checks should be coming. She apologized.
The whole process took some work (looking for a number, I found the initial number I called on your website) and then being on the phone. Probably about 20 minutes total.
