South West Fiduciary negligent and criminal Glendale Arizona

 Southwest fiduciary was hired to assist a man who was admitted into a nursing home to apply for Medicaid. SW lied to Mr. Springfield and to the nursing home claiming that they had filed the application for Medicaid when in fact there is no application on record with the state of Arizona. SW failed to keep promises of coming to see Mr. Springfield and go over what has been done on his behalf. The nursing home was forced to issue Mr. Springfield a dishcarge notice for non- payment of services. The fiduciary has been neglegent and has swindled Mr. Springfield out of what littel money he has and nothing but empty promiss in return. This has casued a great deal of stress on Mr. Springfield and has had a negative impact on his already frail condition.
