United Concordia Dental Insurance failure to pay high option dental claim Harrisburg Pennsylvania

I had high option dental insurance with United Concordia.  I was paying over 40$ a month.  2 dentist confirmed need for a crown.  Concordia will not pay.  I've been fighting for a year as of 12/22/17.  At one point, they refused payment because "peridontal disease was not addressed" and I've never had peridontal disease!  Other times they denied payment because exrays were not received, yet they were sent by my dentist on multiple occasions.  At one point, the would not even respond and I had to contact my state insurance board who I am now sending a second complaint to for help.  I am also sending a complaint to BBB.  I've read multiple complaints against them and they all echo mine.  I'll help whoever and in any way I can to get justice and stop this insurance from fraudulance.
