We were shopping for health insurance and contacted by a representative/agent by the name of Tyler Lane (probably not his real name), an insurance broker, (888) 723-1849 of the George Steele Insurance company who was selling Health Option One's insurance and representing it as Patriot Health.  Mr. Lane deliberately misrepresented the plan as including co-pays after five doctor visits and stated that after the coverage deductibles were met there would be 75/80% insurance company and 25/20% on us cost share.  Also he stated it was a requirement to purchase life insurance to obtain the low monthly insurance rate.  The life insurance coverage is from Fidelity insurance.  I find no requirement in the Health Option One disclosures to purchase life insurance.  The application fee was $99.00.

After receiving the disclosure from Health Option One it it clear the consumer is 100% responsible for all charges in excess of 5 doctor visits and contains clear limits on the amounts for surgical procedures and hospital visits.

When I asked for a detailed disclosure of the plan Mr. Lane indicated it would be made available via email after the application is accepted.  The key concern/complaint here is the verbal representation of plan did not remotely match the actual disclosures from Health Option One.  And Mr. Lane did not even reference Health Option One and only mentioned Patriot Health and Fidelity Insurance.  Further Mr. Lane indicated the insurance companies were AAA rated by the US Government.

Be aware of any agents or brokers representing Patriot Health, Health Option One or Fidelity Insurance.  I am in the process of attempting to obtain a full refund of the payments made to these companies.
