This is a Phishing Scam to obtain your personal medical information, address and phone number.
(The card does not give you any discounts, it just gives them your personal information!)
"At the end of their long "NON-PRIVACY" discloser they indicate you are giving them consent to do whatever they want with your information and not even store it outside of your country."
(THROW IT AWAY, if it seems to good to be true it is normally a SCAM!)
