Vivint and perfection collection my wife signed me up for a security system and signed my name and vivint has been trying to hold me accountable lehi utah

In 2015 i noticed that vivint put something on my credit report, I called them and they said that i had signed up for a home security system in February of 2013.  I told them that I hadnt because my wife and I were seperated in january of 2013 and I wasnt living at the house.  If you look on the order form it confirms that Cristy Moody made the order, not me.  I see my signature, or something like it, I believe that either my xwife, or vivint somehow forged that signature, because I moved out in january of 2013,  I never went back to live there and i didnt order the system.  in 2016 vivint took this off my credit report because I proved my case, but recently they put it back on and are letting perfection collection come after me.  This couldnt come at a worse time as im trying to refinance my house.  Vivint and perfection collection are trying to get an innocent man to pay $600 dollars for something he didnt order.  Its pure thievery.
