Originally my insurance was billed for this supposed top of the line tens unit:
7/16/2011 Home Med Equip ZYNEX MEDICAL INC $1,995.00
Then every month this company sent me 4 sets of sticky pads (leads) to connect to the tens unit that I purchased through them. Every month they billed my insurance for these amounts.
HOME MEDICAL EQUIPMNT 06/16/2011 $249.00
SUPPLIES 06/16/2011 $170.00
SUPPLIES 06/16/2011 $39.96
This last month they billed my insurance company under a different name Stroke Recovery Systems:
12/16/2011 Supplies MEDICAL STROKE RECOVERY SYSTEMS $209.96
12/16/2011 Supplies MEDICAL STROKE RECOVERY SYSTEMS $60.08
These last charges were charged as out of network and stating I had a patient balance owe of $60.08. They were billed as out of area providers and paid at a different rate and leaving a balance for me. I should owe nothing as all my coinsurance and deductibles had been met.
They have totally overbilled my insurance company with substandard supplies and this needs to stop. I don't know how to inform my insurance company of this issue.

Pick up the phone, dial their number and ask to speak to their fraud department.
What am I missing?
