This company faxed us two sets of bills for a Kevin Batesman out of Collon UT in the amount of $6,000.00 for "ONE LIST OF Business records RANDOMLY PULLED - NATIONWIDE sent directly to CLIENT FOR UNLIMITED USE. Total Records delivered on time data ONLY".

An application for credit was attached. The "invoice" looks like a standard form from a word processing program where you just fill in the blanks. Invoice has a date of 04/22/04 with an Inv. # 0981982B.

There is no phone or fax number for Forsythe. This looks like a scam to get credit card information from a con artist.
