I sent a Cooper Vision 30$ rebate to :
Lens Rebate Offer 05-40235, PO Box 540007,
El Paso, TX 88554-0007
The requirement was :
The four (4) colored box-end panels that list your prescription (example: 8.6/14.2)
The box panels are made of cardboard and cant fit in an envelope .There are 4 such boxes for a year supply of lenses. So I peeled off the top layer from the cardboard with all the information and sent it to them. (A cardboard has several layers of sheets of paper).The peeled off panel has all the information like the prescription information and can easily fit in an envelope.I have also sent my dated eye exam receipt and doctors prescription .I don't see a point in sending the recycled empty cardboard on the lower layers.
Now the company is denying my rebate stating :

They needed the box end panels with the prescription information which they have it and not the box top .
