2. Order services between June 1-, 2012 and July 2, 2012 and activate services within 30 days of the order state. Order done on 6/25/201,2 and installed on 6/30/12. Certified mail document proof on 8/4/12.
3. lnclude a copy of dated OfferWire order confirmation showing services ordered. Certified mail document proof on 8/4/12.
4. lnclude the first 2 months bills from Time Warner Cable reflecting activation. Certified mail document proof on 8/4/12.
5. Mail (postmark) within 90 days of the date of order. Certified mail document proof on 8/4/12.
