On October 10, 2008, I stepped into my local UPS store, like I have done numerous times. I stepped up to counter with my box that was already packaged correctly by DELL INC, and proceeded to talk to MATT about the right service that I needed to ship to an APO.
We discussed the different types of shipping methods and different types of insurance coverages. I felt very confident with MATT. So he asked if I would like extra stuffing the box with the camera, I said yes that way I could also put a few pieces of mail in with in...Trying to kill two birds with one stone. So I thought.
I was MATT puppet I signed the forms and signed everywhere I was told to sign, just like always. The form that I signed was the Custom Declaration and Dispatch Note by the United States Postal Service...No problem right?!
I had to remind MATT that I wanted Insurance on the package...the camera is worth $4,500 USD...The first price that he gave me was not with the insurance and that price was for $32.25 which is on the form, he explained what the coverage was and everything. So he added the insurance,so I thought...The price jumped from $32.25 to $53.75 which is the price on my UPS receipt.
The package has never been delivered to the addressee and after looking at the form I realized that the section where the insurance coverage goes its BLANK....MATT is to blame for this....I have called UPS and Unites States Postal Service asking for and investigation, and have came up short...
Its sad because this camera is hobby of US Military man, this hobby gives him sanity in such a hectic environment...He is stressed and Im stressed, I don't know what to do. Of Course they are claiming that it is not insured and Customs is claiming to never have the package...
They have failed me in such way...They will not explain to me why there is a price change on my receipts...I know this package was insured I paid for the insurance and now I get nothing...Not to mention the Parcel Indemnity Coverage, I have called several times and nothing has happen..What oh what shall I do...

You cannot insure a package going to a military address. (APO) What happens with packages shipped to APO's? They are sent to military warehouses and then redirected to where the recipients has been stationed. It can actually take months for the package to get to the intended recipent. Never ship anything to an APO that requires the package be confirmed as being sent, or is not replacable as these packages can and do become lost in the military's forwarding process?

Don't send expensive stuff like that through the post office PERIOD. Should have just sent a few disposable digital cameras.

Your problem seems to be with the UPS Store and NOT the United States Postal Service(USPS). So dealing with the USPS without proof of insurance is probably not going to go anywhere.
Now, APO Mail can take several weeks by Parcel Post especially if you have to deal with Customs. But even after two months it "should" have arrived by this method, hopefully it will eventually show up.
If the UPS Store is refusing to show you how they got that calculation then force them to do it. File a Small Claims Case. In Court they will have to show how they got the amount they charged you. If they can't do that, or you can convince a judge you felt that you did pay for insurance they may be held liable. Small Claims Cases are cheep to file and you should be able to do it for under $50 and a couple hours of work. Depending on the exact situation you still may not prevail but if you do it may be the quickest way.
You are helped by the more paperwork you have. That means you need to keep everything they gave you at the UPS Store. Now, one problem is what exactly did you pay extra for. Based on your numbers it seems you would have been undercharged for USPS insurance. For $4500 in insurance it should have been about $44.50, where you only paid an extra $21.50.

It seems like you used UPS, though your title mentions the USPS. If you used UPS and after filing a lost package report they couldn't find it they should reimburse you for the value of the product, especially since it was insured. If they still refuse to reimburse you try goin up the chain of command.
First ask to talk with a supervisor they have more authority to assist you. If that doesn't work try this suggestion I found: "Each region of the country has account executives who will deal directly with you on a personal basis. It may take a day or so before they call you back. Be sure to get a name and a direct phone number and call back, Call Back, CALL BACK. Try to be friendly and pleasant. It's not their fault the package was lost. They're trying to help you find it. Ask them who you can send a written complaint to, write it, send it, and call them too. The more people on your side the better."
I am sure after all this you'll get your item or reimbursment. If not check into your areas laws and see what legal recourse you can take. Make sure you keep all receipts.
